The Advancing End-of-Life Care Access Award recognizes provider initiatives that have significantly increased the type, scope, or range of services offered to and used by a traditionally underserved population.

Circle of Life Awards

Applications for the 2013 Circle of Life Awards: Celebrating Innovation in Palliative and End-of-Life Care are now available at aha.org/circleoflife. If you have an innovative palliative and/or end-of-life care program, please consider completing this application.  If you have submitted or previewed the application in the past, please review the newly-revised application. Questions and data requests have been significantly streamlined.  For any questions on the award or the 2013 application, please email circleoflife@aha.org.

The awards are currently supported, in part, by the California HealthCare Foundation, based in Oakland, California, and the Archstone Foundation.  Major sponsors of the awards are NHPCO and the National Hospice Foundation, the American Hospital Association, the Catholic Health Association.  The awards are cosponsored by The American Academy of Hospice and Palliative Medicine and the National Association of Social Workers, with additional support from the Hospice and Palliative Nurses Association.

In 2012, up to three organizations will win Circle of Life Awards; additional organizations may receive Citations of Honor. Awards and citations will be presented at the American Hospital Association Health Forum Summit, July, 2012.
