Beginning in December of 2008, Medicare‐certified hospices have been required to implement an ongoing quality assessment and performance improvement (QAPI) program that must demonstrate measurable improvement in palliative outcomes and services and must address adverse events.  Key to a successful QAPI program is completion of performance improvement projects (PIPs) that lead to measurable and sustained improvement in performance.  This webinar will provide a how-to guide for carrying out a PIP, using two case studies that will take participants through the PIP process, from identification of the topic/area for improvement to an end point where measurable improvement occurred and could be monitored for sustainability.

Vivian Dodge, RN, BSN, MBA

Vivian is the Director of Quality Management at Hospice of Palm Beach County, Inc. Vivian has thirty years of progressive healthcare industry experience; with ten years hospice experience. Vivian joined the staff of Hospice of Palm Beach County, Inc. in 2003. Vivian has a proven track record for regulatory compliance and ensuring accreditation for the organization and demonstrated expertise in QAPI programs. Vivian has been a Certified Health Care Risk Manager for the Agency for Health Care Administration (AHCA), State of Florida since 2007. Vivian frequently speaks on QAPI and regulatory compliance topics for Florida Hospices and Palliative Care, NHPCO and other healthcare organizations. Vivian has been a member of the NHPCO Quality and Standards Committee since 2008.

Lin Simon, PhD RN CHPN

Lin is the Director of Quality for Gilchrist Hospice Care serving central Maryland. In this role since 2009, Lin has used both Lean and Institute for Health Care Improvement methods to improve patient experiences and safety outcomes in Gilchrist hospice and in the larger GBMC Health Care System. Before coming to Gilchrist, Lin worked as the Quality and Research Specialist at the National Hospice and Palliative Care Organization leading Quality Improvement Collaboratives for hospices across the nation. Lin's quality expertise is in teaching quality improvement methodologies and coaching improvement teams to successful outcomes. Lin received her Masters in Long Term Care and Doctorate in Philosophy in Ethics, Administration and Health Policy from George Mason University in Fairfax, VA.

John Thoma

John began his career with Hospice of Wake County (HOWC) in 1991 as Director of Finance and Information Systems.  He was appointed Executive Director in 2002 and CEO in 2004. Hospice of Wake County provides hospice, home health and palliative care in a seven county region and operates a 20 bed hospice inpatient & residential facility and a community grief center.  HOWC has participated in numerous projects to improve end-of-life care including a University of North Carolina study on improving care in nursing homes and the PEACE project.

John serves on the NHPCO Board of Directors, chairs the NHPCO Quality and Standards Committee and the Governance Committees and serves on the Membership Committee.  He is a graduate of Boston University where he received a Bachelor of Arts degree concentrating in Spanish.  He also attended Tufts University Graduate School of Arts and Sciences Non-Profit Institute concentrating in Community Economic Development and Non-Profit Management.

Webinar Objectives

At the completion of this Webinar, participants will be able to:

  • Outline the essential elements and  basic steps in a Performance Improvement Project (PIP);
  • Describe how to plan, develop and implement a PIP;
  • Identify multiple sources for PIP data collection and methods of analysis;
  • Explain how to integrate PIP results into practice to sustain improvement.
