It is imperative that all hospice staff recognize the importance of the plan of care. After completing this course, staff will realize the necessary plan of care components for hospice patients that reside in facilities and in the home, comprehend the requirements for the initial and comprehensive assessments, understand the required plan of care changes when patients are transferred to different levels of care, understand the cycle of care and the role of the interdisciplinary group (IDG) in care planning.
