NHPCO has added an annual national data collection Palliative Care Supplement to our National Data Set.  This effort was developed in response to an identified need due to growth in enhancements to palliative care in hospice programs. We are instituting this data collection initiative to provide hospices with the same invaluable organization level comparative data for palliative care programs that we provide for hospice programs. Thank you in advance for contributing to the National Data Set Supplementary Survey on Palliative Care.  The information you provide supports the growing hospice and palliative care body of knowledge.

• Use the 2011 NDS Palliative Care Data Submission Worksheet as a worksheet to compile your data.

• NOTE: Access to the Palliative Care Supplement requires logging in with your individual NHPCO member ID and PW, not your DART ID and PW. 
• Hospices that are not current members of NHPCO can also submit data. While there is no fee to participate, membership is required. For more information about submitting data as a non-member hospice, please send an email to NDS@nhpco.org.

The submitted data will be compiled in an annual national report on palliative care that can be used to compare organization level program characteristics, to define strategic goals, and to set operational targets for your palliative care program.
