Family Evaluation of Palliative Care (FEPC)

The Family Evaluation of Palliative Care (FEPC) is a post-death survey that captures family members’ perceptions about the quality of the palliative care that their loved ones received – whether that care was provided by a hospital-based consult service or by a hospice program offering palliative care. The questions on the FEPC survey are based on those in the Family Evaluation of Hospice Care (FEHC) survey, with wording modifications appropriate to palliative care service delivery.

To begin submitting FEPC data, login to the NHPCO DART System using your hospice's NHPCO DART ID (formally known as Provider ID) and DART Password (formally known as Provider Password). If you do not know the DART ID and Password for your hospice, please contact NHPCO Member Services at 1-800-646-6460.

Step 3 - Reports
To access your FEPC Reports, login to the NHPCO DART System using your hospice's NHPCO DART ID (formally known as Provider ID) and DART Password (formally known as Provider Password). If you do not know the DART ID and Password for your hospice, please contact NHPCO Member Services at 1-800-646-6460.
