Quality Reporting Important Data Collection Notice: (Summarized from the CMS User Guide)
After the webinars presented by CMS in April 2012, two important changes were made to clarify patient inclusion for the reporting period and the timeframe for the follow-up question. Here, we highlight the “Need to Know” facts for these changes.
- Hospices must ask patients the follow-up question by midnight of the third day or the patient will be considered lost to follow-up. (User Guide page 25)
- Also, to be included in the first reporting period for NQF #0209, a patient must be admitted no later than 11:59 PM on December 26, 2012. (User Guide page 30)

The Measures (formerly EROM) include the following patient-centered measures:
- Comfortable Dying/NQF #0209 (pain brought to a comfortable level within 48 hours of initial assessment)
- Self-Determined Life Closure (unwanted hospitalization; unwanted cardiopulmonary resuscitation)
- Patient Safety (falls; infections; adverse events) coming soon
