Patient Outcomes and Measures (POM)
- NHPCO accepts POM data on a quarterly basis for both the Comfortable Dying/NQF #0209 Measure and the Self-Determined Life Closure Measure.
- NHPCO does not submit data to CMS.
- Hospices should now be collecting data on Comfortable Dying/NQF #0209 and the Structural QAPI measure for CMS from January 1, 2013, through December 31, 2013, for the FY 2015 payment determination. Hospices will submit this data to CMS by April 1, 2014.

The Measures (formerly EROM) include the following patient-centered measures:
- Comfortable Dying/NQF #0209 (pain brought to a comfortable level within 48 hours of initial assessment)
- Self-Determined Life Closure (unwanted hospitalization; unwanted cardiopulmonary resuscitation)
- Patient Safety (falls; infections; adverse events) coming soon
