Nominees must file a completed nomination packet no later than close of business July 31, 2022. Download and review the General Board Member Expectations.

Please keep in mind, the Governance Committee will not consider incomplete nomination packets.  It is the responsibility of the nominators and the nominees to ensure that all materials are complete, accurate, and submitted by the deadline.

Nomination Packet 

Nominees may use the Nomination Packet Checklist to ensure all documents are completed before submitting to NHPCO.  A complete packet must include the following items:

      • An Acceptance letter from the nominee indicating their willingness to serve. See sample letter on page 2 of this packet.
      • Two letters of nomination from NHPCO provider members, addressed to: Chair, NHPCO Governance Committee. See sample letter on page 3.
      • An Endorsement letterfrom the CEO or Chair of the governing board of the nominee’s program or company. Address letter to: Chair, NHPCO Governance Committee (see sample letters in this packet for address details).
      • A Biographical Profile, see pages 4 – 5.
      • A completedNominee Questionnaire, see pages 6 – 7.
      • Advance Interview Questions, to be completed and submitted with Nomination Packet, page 8.
      • For the purposes of conducting a background check, a completedBackground Check Release Form. NHPCO requires nominees for the NHPCO board to undergo a background check to confirm their educational and employment history, as well as identify any criminal convictions. See page 9.
      • A current copy of nominee’s curriculum vitae or resumé.

All of the above materials will be accepted via email and in Word format only, except for the acceptance, nomination, and endorsement letters. Please send completed nomination packets to Maura Ward at boardnominations@nhpco.org.
