This course was originally presented at NHPCO, AAHPM and HPNA’s 2015 Virtual Conference entitled Clinical Advances in the Art and Science of Care.

To be compliant with current regulations, hospice clinicians need to evaluate patients’ multiple chronic morbidities to decide which have an impact on prognosis. Since no condition is isolated, there are layers of diagnoses, interacting in various ways, in turn causing adverse impact on prognosis and patients’ symptoms. This session will offer a guide to evaluate patients’ diagnoses and morbidities, to determine how they affect prognosis and how to determine relatedness of various interventions.

Objectives:

  • Distinguish the various chronic medical conditions and morbidities with which patients with life-limiting illness present
  • Demonstrate how the layers of morbidities interact to adversely  impact on patient quality of life and prognosis
  • Propose a summary of the adverse diagnoses and prognoses so that  related interventions become evident

Joel Policzer, MD, FACP, FAAHPM

After training in Hematology-Oncology, Dr. Policzer started a community practice of the same. Along with this he began a part-time collaboration with VITAS lasting 14 years, transitioning to full-time 16 years ago for a total of 30 years working in hospice and palliative medicine, in both clinical and administrative positions. His current position involves developing educational programs and teaching medical students in both didactic and clinical settings, as well as developing educational programs with professional societies and industry partners.
