This session was originally presented at NHPCO’s 2014 Virtual Conference 'Hot Topics in Palliative Care.' Sharp HealthCare is transforming the healthcare experience for patients living with advanced chronic illness by using evidence-based medicine and data driven results.  Building a collaborative partnership with key medical groups and hospitals, Sharp Hospice Care has found ways to better manage patients with chronic heart failure, dementia, COPD, advanced stage cancer and other diseases by bringing care to the place where the patient and family reside – to their home.  Transitions bridge the “four walls of traditional care” by expanding the principles of palliative and hospice care at an earlier point in the patient’s disease process.  Data strongly support the efficacy of the program through positive outcomes in both quality care and financial performance.

Suzi Johnson, MPH, RN

Suzi Johnson is Registered Nurse with more than 25 years of experience in healthcare. She currently serves as the Vice President of Sharp HospiceCare, which is part of Sharp HealthCare, San Diego, CA. Sharp HospiceCare was the recipient of the AHA Circle of Life Citation of Honor in 2010. Suzi’s responsibilities include strategic planning, business development, operational oversight of the hospice, two hospice homes, community outreach and philanthropy. Her extensive experience in leadership and her passion for excellence in end-of-life care has helped to make Sharp HospiceCare the first hospice in San Diego to offer a weekend bereavement camp for children, a Memory Bear Program, and two hospice homes specializing in end-of-life care.  In addition, Suzi has been the visionary in developing a home based palliative care consultation program for advanced illness management of chronic diseases such as heart failure, COPD and dementia.
