This session was originally presented at NHPCO’s 2014 Virtual Conference ‘Hot Topics in Palliative Care.' This session will describe Innovative Models of Palliative Care Delivery in the context of community based hospice and palliative medicine.  Discussion will center about the ways in which these models can extend the reach of hospice and palliative care programming and give patients and families who are coping with serious and complex illness access toes to essential services across the continuum of care.

Objectives:

  • Review innovative components of a community based Palliative Care and Hospice program within the context of an integrated delivery system of health care
  • Identify barriers and opportunities in developing such models
  • Define pathways and strategies for program development
Faculty:

Stephen Radwany , MD, FACP, FAAHPM

Dr. Radwany has been Medical Director of Hospice and Palliative Care Services at Summa Health System since 1999 helping to establish a hospice program, palliative care consult services and palliative care inpatient units while serving as Program Director of its Hospice and Palliative Medicine Fellowship.  Dr. Radwany has been active at the Northeastern Ohio Medical University (NEOMED) where he is a Professor of Internal Medicine. He has presented at many meetings throughout the USA as well as in Cuba, Hungary, Peru, Vietnam and Argentina. He is a Fellow and Laureate of the American College of Physicians and a Fellow of the American Academy of Hospice and Palliative Medicine, has a Certificate of Added Qualifications in Geriatric Medicine and has been board certified in Hospice and Palliative Medicine since 1997. Dr. Radwany received his medical training at the University of Cincinnati and Northwestern University.
