This session was originally presented at NHPCO’s 2014 Virtual Conference ‘Hot Topics in Palliative Care’. "At Home Support", a program developed by Hospice of Michigan,  is an advanced illness coordinated care solution focused on providing services to the 5 percent of Medicare patients in an ACO or Medicare Advantage network that account for 50 percent of the cost.  These patients typically have Stage IV chronic illness with comorbidities and an expected median life span of 24 months. Developed through 12 years of evidenced-base outcomes research, "At Home Support" improves quality outcomes and patient experiences, while supporting stressed family members. In addition, it’s saving saving 30 percent net total health care costs for this 5 percent cohort. These savings are primarily produced from substantial decreases in unnecessary ER utilization, hospitalization and re-hospitalization.

Michael Jasperson, MBA, MSA

Michael Jasperson is currently the Vice President of Marketing and Business Development and Chief Marketing Officer for Hospice of Michigan. He has responsibility for corporate marketing efforts statewide. He also has accountability for Hospice of Michigan’s dedicated Inpatient Hospital service teams. Prior to Hospice of Michigan, Michael spent 11 years with Blue Cross Blue Shield of Michigan. His background at Blue Cross Blue Shield of Michigan included positions in Strategic Planning, New Product Development and Sales Leadership. Michael earned a master of business administration (MBA) from University of Tampa, with a concentration in Health Services Administration. He also holds a master of science in administration (MSA) from Central Michigan University. Michael received his baccalaureate degree from Michigan State University.
