CMMI’s Primary Care First Model – Seriously Ill Population (SIP): Overview and Request for Applications

This webinar (originally presented 11/15) focuses on the recently released Request for Applications (RFA) for the Primary Care First- Seriously Ill Population model. The application period for practices applying to begin participation in the model in January 2021 opened on October 24, 2019 and will close on January 22, 2020. We will provide a detailed overview of the RFA and how hospices and palliative care organizations can be best prepared to apply and participate. We will also explore hospices readiness to provide SIP programming as well as the specific areas for providers to address to take advantage of this opportunity.
