CAHPS® Hospice Survey

The Centers of Medicare & Medicaid Services (CMS) has developed a post­death family caregiver survey to systematically assess patient and family experiences with hospice care.  The survey is part of the Consumer Assessment of Healthcare Providers and Systems (CAHPS®) family of surveys and is officially designated by CMS as the CAHPS® Hospice survey.

The survey is 47 questions long and is available in English, Spanish, Chinese, Russian, and Portuguese.  The Chinese mail survey is provided in both traditional and simplified characters and targets both Mandarin and Cantonese speakers.  

Surveys responses will be used to calculate 11 quality measures:

    Composite Measures

  • Hospice Team Communication
  • Getting Timely Care
  • Treating Family Member with Respect
  • Providing Emotional Support
  • Getting Help for Symptoms
  • Getting Hospice Care Training (Home Setting of Care Only)

    Single Item Measures

  • Providing Support for Religious and Spiritual Beliefs
  •  Information Continuity
  •  Understanding the Side Effects of Pain Medication

    Global Measures

  • Overall Rating of Hospice
  • Recommend Hospice

The survey is administered by CMS approved vendors on behalf of hospices. Hospices are required to contract with an approved survey vendor and to provide caregiver contact information to the vendor on a monthly basis.

Hospices with fewer than 50 decedents during the prior calendar year are exempt from the data collection and reporting requirements for payment determination.

What’s New

  • Information for Hospices Webpage has been Expanded (05/03/2016)... A new navigational button has been added containing helpful information and links intended for hospices and other interested parties regarding the CAHPS Hospice Survey. The Information for Hospices navigation button now has three sections:

  • 1) The General Information page provides quick reference links and information related to the CAHPS Hospice Survey;
  • 2) The Podcasts for Hospices page contains a series of podcasts with key items related to the CAHPS Hospice Survey; and
  • 3) The Hospice-specific FAQs page contains frequently asked questions that may be helpful for hospices and other interested parties pertaining to the CAHPS Hospice Survey.
  • Vietnamese CAHPS Hospice Survey Mail Materials (05/03/2016)..... The CAHPS Hospice Survey Project Team has posted the Vietnamese translationof the mail survey materials and the Consent to Share Responses supplemental question text. These documents are found under the Survey Instruments navigation button.

    Survey vendors are permitted to administer the CAHPS Hospice Survey in Vietnamese beginning with March 2016 decedents. The CAHPS Hospice Survey materials in all translations will be updated to include “Vietnamese” as an answer option for Question 47. CMS anticipates providing these updated materials in the next few weeks. In addition, the CAHPS Hospice Survey XML File Specification Version 2.2 will be released in the next few weeks to include “Vietnamese” as a Valid Value for the “Language” and “CHomeLang” fields. Please note that the XML File Specification Version 2.2 will be implemented for the August 10, 2016 data submission deadline.
